  1. I am tryung to create a survey with those radio buttons 1) very satisfied 2) satisfied 3) neutral 4) unsatisfied 5) very unsatisifed.

    Is there a way to create those radio buttons and have them filled in automatically when users click on it? I cant seem to find them in 2003 Word.

    This is still not quite an answer.

    Are you trying to create an html file (web page) in Microsoft Word 2003 (Word)? Are you creating a Word document with macros that allow you to fill out a form?

    If it is the latter, you go into Tools->Macro->Visual Basic Editor, then click on the Design Mode button (about sixth from the right).
    You'll need to know a little bit of VB to get these running right.

    No. the control itself is a macro command. Regular Word documents aren't meant to be interactive, just like any text document.

    What exactly is your goal? If you are making a fillable form, try Adobe Acrobat (PDF), which is more portable. Word is too proprietary in that respect (macros & visual basic).

    You may also want to try what was suggested: a web page. You can find out how to do what you want here.
