Assuming the original file(s) are OK, it is typically the conversion process (ie: tool used) that creates the out of sync issue and you may end up needing more than a single tool depending on the various types of source files you are using.

You can look in the Tools section for a list of apps that can be used for iPod conversions (Portable/Mobile/PSP/iPod).

I use Videora mostly. Occasionally I will get an out sync video. To get around this I have created a couple of fixed framerate profiles which generally solves the problem.

I've tried a number of programs and had the best luck using ADS Instant Video To-Go.

A good little payware program is from Jesterware. No, I definitely wouldn't archive these conversions, but if you want a quick reliable, no-nonsense, working file that plays solidly on your iPod, you shouldn't go wrong. Been using it over a year now.

Nero Recode is very good too.

If you're converting directly from DvD, then HandBrake is ultimate. Free too.

BTW - I personally believe, that if you're only encoding for iPod, not archiving, stick with the older, and quicker-to-encode-to, MPEG-4. With all resources involved in H.264, long encoding times, and buggy tools still for it, is just not worth it for a program you'll watch then delete on your iPod - even if the quality is better. Who cares? It's an iPod, not a big screen.
I hate VHS. I always did.
