Out by a constant amount, or does it drift out as the movies goes on ?

Out by a constant amount - determine the amount and add a delay to the audio to adjust. Common practice on commercial discs.

Drifts out over time - something got screwed up during the encoding.

Load the video and audio into Media Player Classic, either as a program stream or as demuxed elementary streams. Set it playing, then use the + and - keys on your numeric keypad to vary the delay in real time until you get it lined up. Note the delay displayed at the bottom of the screen, and use this to adjust when authoring.

You can use delaycut to just alter the AC3 file by your delay amount, then author with anything. I use DLP, which has this feature built-in.
