Probably by using MP4box to demux it first from a mp4 file.
I haven't tried that but some time ago I remember reading that this option was implemented and that it was possible to demux .idx/.sub file.
If it works, then you can use Subrip to convert .idx/.sub to .srt format.

    EDIT:
I have just tried it and it works.
You have to download YAMB which is a GUI for MP4box.

Apologise if I am replying an old thread but it seems Jeremiah58 has a solution for my problem. I have a scientific documentary in mp4. It has a sub printed in , I would like to extract it. I have used Yamb and subrip. With SubRip I can use only VOB files (I created it using nero on a DVD. Even that SubRip doesn't work
    There is a solution? Thanks
