I found this at the Afterdawn Forum: http://forums.afterdawn.com/thread_view.cfm/250986 from manicalic:

Three separate ways to have subs in .rm(vb) files:
    NOTE: all require Real Alternative
IMPORTANT: before installing RealAlt you MUST uninstall Real Player (will not work anyway)
NOTE2: Subtitles file has name exact as movie, extension is appropriate for subtitles format.

    1. open file in player supporting subs (Bestplayer etc.)
    2. Media Player Classic:
Options > Playback > Real Media Video > (choose) DirectX 7/9
    Can now (auto)load subtitles
    3. Installed DirectVobSub: just change extension from .rm(vb) to .avi (not convert, just change filename), DVobSub will load subtitles automatically.

3rd way is the only that works in Windows Media Player.
It sounds like it should work. I would try .srt subtitle format.

There is a 3rd party tool that allows you to mux subs into a rmvb file. Then you definitely need Gabest's splitter for playback though. Then again mplayer may work. Officially I think subs are supposed to be loaded via SMIL for rm/rmvb files. Should work fine in Real Player that way.
