how can I achieve the fade out of an image in photoshop. See example

1. Create a white layer above your image
2. Click on that layer, then click on the Add Mask icon on the layer palette
3. Click on the mask itself on the White layer
4. Click and hold on the paint bucket (fill) tool until the slide out appears, and select Gradient.
5. Make sure you have a Black to White gradient selected, and drag across the image to put a gradient into the mask. Experiment with direction and length of gradient until you have the effect you want.
If you google you will literally thousands of great, simple photoshop tutorials. This is about as simple as masks get.Read my blog here.

STEP1: That image has been stripped down to 3 colors. There are various ways to reduce an image color palette.
STEP2: Next, the hues are swapped 4 times to arrive at that image.
STEP3: Then they are collaged.
