You would have to decrypt it first if you wanted to remove the protection from a commercial DVD. DVDFab Decrypter can do that or RipIt4Me or others. Then you should be able to make a ISO from that file. But if the image file were of a commercial DVD, you would have to use a dual layer disc if you wanted to burn it.

DVDDecrypter > ISO Read should do what you need for most DVDs.
If in doubt, Google it. -
If you cant fit the original onto a single layer disc and dont want to buy or dont have a DL disc then you'll need to rip the DVD using the tools mentioned or good ole AnyDVD and use DVDShrink to transcode the video down to size.
Also if you want to create an ISO from DVD Video files (VIDEO_TS and AUDIO_TS folders) then IMGTools Classic will do this for you with great ease.~Luke~
