  1. if you have windows xp you can find and run msconfig and edit your boot loader there. Some people install a 2nd windows on another hard drive in case something happened and add it to boot loader in case of corrption or HD failure.

  2. Did you delete all of the files out of Windows/Documents and Settings/whateveryourusernameis/Local Settings/Temp? I've had some stuff in that folder that recreated trojan files. I would also look in Windows root folder, System, and System32. One other thing I would do is turn off System Restore before booting into Safe Mode (I use Safe Mode with Command Prompt myself).

    As for software, I use McAfee version 6 (I need to upgrade that), Microsoft Anti-Spyware, Spy Subtract with CWShredder, Hijack This, AdAware, and Spybot.

    I really had a tough virus or trojan once. It required me to turn off System Restore, do a hard shutdown, and then boot into Safe Mode. The hard shutdown was needed because the offending files would rename themselves upon shutdown. I was able to rid myself of this pest successfully without having to reformat.
