UndercoverXP and thin DVD case insert

I just purchased some dvd inserts from supermediastore.com for thin dvd cases. I have been using undercoverxp to print out the covers on my old inserts. The new inserts are a different size. Does anyone know what settings i need to usein undercoverxp for an insert this size:

8.5 x 11 (us legal) page size

16mm boarder on left and right sides of page
9mm border on top and bottem of page
the spine of the insert is 138mm from the top and bottem edge of the paper.

seashadow i dont buy the cover and more i print my slim covers on my epson r200 and i use epson glossy photo paper they look great. i brought a little paper trimmer at staples for $10.00 to trim off the excess. paper cost me $34.00 for 100 sheets
