I've had my trusty Samsung S-224 for close to two years now, but over the past few months it's decided to skip, jump, and freeze just about every DVD I play in it.

Could cleaning the lens help, or is it simply just giving up on me now?
    Thanks.

    yup could be either.

(if you have changed media recently could be that)

It is always worth having a lens cleaning disc around. After cleaning the disc itself it is the next easiest and cheapest and most sensible thing to try.

Yes it could be your drive in its later life, but dont condemn it until you are sure.
