I have a Lite-On LDW-411S which has worked great until recently. All burns are failing regardless which software I use. I'm using the same software and media that I used when burns were succeeding.
I'm wondering if anyone with a bit more experience can tell me what might have gone wrong based on the log I'm including. The errors appear to be at the end of the log, but I don't understand how to read them.
One odd thing I'm noticing in the log is that it's saying I'm using Nero 6.3 when I'm using 6.6.0.5.

Bazooka is right, so don't throw the burner out the window yet.
Discs often tend to go bad toward the outside. Keep your file size at (or below) 4.2GB and try a different brand of disc like Maxell, Fuji, & TDK. -
If it's reporting the wrong Nero version, it could be a DLL conflict, or a registry glitch, or any number of other such problems.
Try completely uninstalling Nero -- including going into regedit and removing all keys under HKEY_LOCAL_MACHINE\Software\Ahead and HKEY_CURRENT_USER\Software\Ahead (if any still remain) once Nero's uninstall is complete, then deleting the entire C:\Program Files\Ahead directory (if it still exists after the uninstall).
Once you're sure no traces of the program remain, reboot the system and re-install Nero from scratch, and see what that does for you. -
1. Do a defrag
2. Dont do stuff on the pc while you are burning
3. slow down the burn
4. Try using DVDecrypter instead of nero for your burn. Personally, I would just use dvddecrypter, but I am suggesting using dvdecrypter to rule in/out nero -
flash and convert your liteon 411 drive to a liteon 811 with the zebra mod patch....check the LITEON FORUM at cdfreaks.com
