Where to get / how to make cool transitions

Where can I find cool transitions like you see in football games and other live tv shows? Is it possible to make these?

If you've got a good imagination, a good video and image editor, you should be able to create anything you want.

I create 95% of transitions and 100% special effects using Photoshop and Premiere. Tools like Adobe After Effects and Ulead Cool 3D, would also come in handy.

Yes, as a previous poster says, After Effects. Indeed, that's what a great many of those transitions you see on TV are created in.

There's almost nothing it can't do -- the downside is that it's pretty expensive (although if you don't need advanced keying and stabilization you can just get the standard edition, which is about half what the pro version costs).
