I/O errror....What does this mean?

I bought a DVD of a concert (VHS transfer to DVDR) and I want back it up. I've tried everything. I don't know what the problem is. I tried DVDdecrypter and after reading 80%, it said I/O error. Does anyone know what this means?

In Tools->Settings->Device tab select "Fast Error Skip",then go to I/O tab and select "Ignore Read Errors".You might want to clean the disk with Windex too.
