With a DVD it may get a little more complicated, but for general subtitle work, here's a few:
DVDSubEdit
Time Adjuster
VisualSubSync
Subtitle Workshop

Assuming they're out of synch by the same amount all the way through, and moving them forward or backward in time by the same amount will fix them, then DVDSubEdit is the tool for the job, as it can fix them within the vobs (i. e., without demuxing):
