Are you using the same computer listed in your profile? Most likely you don't have the CPU and GPU processing power to decode, playback and keep sync in real time with the 4K files.
MPC is far less resource intensive than Premiere.
Also put mediainfo in tree view to see if your files are actually variable frame rate -- quite common with phones.
Any particular reason you aren't using the latest Premiere Pro CC 2015 (9.1)? -

Probably an issue with variable frame rate - look at the min / max rates

It's a limitation of most phone cameras. Very few if any can shoot CFR.
media players can play back VFR video in sync ( such as those commonly found in phone cameras) because timecodes indicate the display time of frames which change (some frames are displayed for longer or shorter periods of time) . VFR video of this type is actually missing frames compared to a CFR version of the same video . In contrast, most editing software have CFR timelines. They are constantly 29.97, or 24, or some other number. They can't handle frames that display for longer or shorter times. You cannot edit VFR video of this type if it's more than a bit variable (minimally variable deviations can be treated as CFR essentially)
The resolution is usually to convert to CFR. This means re-inserting duplicate frames as to where they were dropped in approximately the correct locations as they would have been. You can use avisynth with DirectShowSource() and convertfps=true for example. This has been discussed many times, just search. It will become mostly in sync in the editor (a lot better than what you have now, but still might be very slightly in/out of sync in some sections), appropriate for editing. -
It depends on what you use.
You usually have to re-encode when using avisynth (UHD uncompressed won't be fun to use ; you need faster storage I/O and more space). So if you use a high quality , lossless or near lossless intermediate format it will look very close or the same as the original. If you use a garbage intermediate it will look like garbage.
