How you do it also depends on what authoring tool you are using. Some require each button to be on separate layers, and sometimes the layers have to be names in a particular fashion or in a particular order. Others are more tolerant and only need one layer for buttons and one for the background.

I use DLP, which requires a single layer for the background, plus a separate layer for each sub-picture
