How to create .mp4 file by using raw .h264 and .aac files for JW Player

I'm a newbie at video editing and I would like to create .mp4 files for JW Player.
After using MKVextractGUI I was able to have raw (.h264 and aac) files from original .mkv file.
Then I created mp4 files by using mp4muxer(mp4creator).
However, I could not play .mp4 file in JW Player after uploading to internet.
I might miss a part during the mp4muxer process.
Do you have any ideas what should I do?

Try using mp4box instead (yamb is the gui). It places the moov atom at the beginning for streaming

I hope mp4box locates MOOV atom to the right place. I also have some concerns about 'yamb' because of the confusing reviews.
