For the past 6 years I have been working on a disc labelling project. It's at the market research stage now, and as engaging a commercial survey company is not an option, I made my own.
Unfortunately, I don't have enough friends (and apparently they don't have enough either; sad really) to get a meaningful number of responses, so I am asking for assistance PLEASE.

It is anonymous and takes less than 5 mins (unless the survey host server is having a crawl, which it has done occasionally, sorry).
Yes, this IS real. Both the actual project, and the promises made on the website. I _so_ want to make them happen...

Well, good luck. You should know however that most people here seem to be of the opinion that adhesive disc labels are a bad idea, and likely to damage the disc and/or player.
If you want a pretty disc, use inkjet printable discs, not adhesive labels. -

I hear you on the stickies. I've had a couple of interesting experiences with them. Some swear by them however, so I reckon they're an option. The commercial format will largely depend on the results of the survey. Right now the choices are: single adhesives (for used discs), pre-printed on blanks and/or a software template (to then print or etch).

I want to amplify what Alan said.
Many years of user feedback strongly indicate that adhesive labels are a very bad idea, also that the problems they pose do not always appear immediately. The precise cause is not exactly known, balance, optics, heat have all been suggested. The extremely high negatives associated with such labels are so strong as to reflect very, very badly on anyone who seriously suggests their use as a commercial venture.
Adhesive labels on DVD-Video disks are Totally Unacceptable. Case Closed, Done Deal, No Questions.
Also, while the questionnaire determined inkjet printing on printable blanks is my preferred method, it did not discover that 90% of my disks have only a simple Sharpie scrawl, due in part to the PITA factor of printing the labels but mainly due to the scarcity of quality printable disks in my local stores. -

just another nay sayer to adhesive labels here. on cdr, they,over time eat the reflective media that is deposited on top of the plastic stamper. on dvdr they cause un-balance of the blank, that at high speed causes wobble. neither is acceptable.
