Digital TV transmission via cable questions

I have a new Samsung TV with NTSC and ATSC tuners. I have standard cable with no STB. I get great HD pictures from unencrypted local channels and PBS. I also get two digital channels promoting Video on Demand.

Occasionally I will find a digital channel with a movie, such as Failure to Launch, playing. There is no identification and it seems like a VOD movie. How is VOD transmitted to the customer over cable? Why does it show up on my TV?

When watching the movie PQ will be great. Then the picture goes into Fast Forward where snapshots are shown in sequence and then it slows down to real time. What is causing this?
