I'm at the bottom of a 50pk of Optical Quantum OQBDR04LT-50 4X BD-R discs where you see the loose round plastic divider/protector. There are two BD-R LTH discs being used as that divider, followed by an actual plastic divider.
Are these LTH discs so undesirable that the manufacturer sees them fit as packaging material?
Disc ID: UMEBDR-114-000 which I searched but cannot find

I just answered my own question. ImgBurn has encountered a I/O Error, which makes this my first bad BD-R burn in over 2 years. Case closed.
Try StreamFab Downloader and download from Netflix, Amazon, Youtube! Or Try DVDFab and copy Blu-rays! or rip iTunes movies!
