Best way to cut out 30-45 seconds of video in an MPEG-2 file

without the need for re-encoding which would cause the loss of even more quality since the original MPEG file is not all that great to start with?

All I need to do is excise a small portion, close the resulting continuity gap and resave as an MPEG-2 file.

Any sugegstions or advice appreciated.
