Please do a search of this forum first. This issue has been discussed a zillion times. The short version: "codec mismatch" means that the ordering of the streams is backwards from what ffmpegx wants. Click on "invert mapping" to fix that one. Other problems can arise, however, so do a search for what to look out for (and how to solve).

If your source file has only video (and no audio), then you should de-select Encode audio. Then the number of input streams will match the number of output streams.

in the audio tab try selecting the "invert maping" check box and make sure nothing is seletec in your options tab. ffmpeg sometimes gets confused on which track is the audio and the video from the source
