Is there any difference visually between x264 q21 and x264 2pass?

It depends on the bitrate or filesize you set for the 2-pass. If you make the 2-pass version come out to be the same size as the 1-pass Q21 (with other factors such as filtering, resolution, audio bitrate, etc. the same), then you shouldn't be able to tell the difference. Some might argue, though (and I'm one of them), that a Q encode will have slightly better quality than a 2-pass encode for the same size.
